Teen Summer Camp Application Procedures

APPLICATION PROCESS:
After receipt and review of your complete application, the admissions team will notify you of your acceptance to the program by email and official acceptance letter via post.

DEADLINE FOR FINAL FORMS AND PAYMENT:
Final payment and forms are due May 1, 2012. For applications submitted after May 1, 2012, payment in full and all forms are required with application. We understand that it may take one to two weeks to get a doctor's signature on the Medical Release form. This form may be submitted as soon as it is completed. Admittance to the program is not final until all forms and payment are received.

DUE UPON ACCEPTANCE:

  • Final Payment: Accounts need to be paid in full by May 1. You may login to your SOCAPA Online Account and pay by Credit Card (MasterCard or Visa Only) at any time or you may mail in a check. We also accept wire transfers (please email our office to request wire transfer instructions).
  • Online Health History Form: This is an online health history form that can be accessed by the primary parent through your SOCAPA Online Account.
  • Online Travel Info Form: This is a simple online form that lets us know how you will be arriving to and departing from the program. If you require an airport transfer, this is where you put in your flight information. This form can be accessed by the primary parent through your SOCAPA Online Account.
  • Paper Medical Release Form: This paper form stating that the student is both physically and mentally fit to attend camp needs to be signed by a Medical Professional. Instruction on how to mail, fax or scan and email the form to our NY office are provided at the bottom of the form.
  • Copy of Health Insurance Card: Along with the Paper Medical Release Form, please provide a copy of the student's Health Insurance Card, front and back, as proof of active health insurance. All students are required to have Health Insurance coverage in the United States for the duration of the camps. For short term travel and health insurance we can recommend Assurance Solutions.
YOUR SOCAPA ONLINE ACCOUNT:
You can manage your entire SOCAPA experience through your SOCAPA Onine Account. You may check your admission status, check which forms we have received, review billing info, make payments and update any account information (travel, medical, address change, etc.).
