- Health History Paper Form - (Only if you CANNOT complete our SECURE Online Health History Form which is available by logging into your secure online account. In the case of an emergency, this allows us to securely access your Health Information from any computer at any hospital. And if you return next summer, you can simply update the form with new info rather than reentering all the data.)
CAMPUS-SPECIFIC INFO PACKETS:
These info packs contain important campus-specific information including a check-in schedule and location, directions to campus, airport pick-up/drop-off info, description of dormitories and a suggested list of what to bring to camp. The 2013 Campus Information Packs are available below:
